Computer Service Agreement Date ___/___/_____
CIO Systems, Inc. provides computer hardware and software support for general business applications. Once a month we will come to your facility and do a general check of your computer systems to make sure they are running normally. Once a quarter we will also to a complete preventive maintenance on your computers. Printers and other output devices are not covered by this agreement, but we will try our best to fix them.
In addition to the monthly service your company is entitled to ____ additional service hours per month. Service hours not used do not roll over to the next month. Included service hours cannot be used for the installation of new equipment purchased from another company. Service calls can be used as credit against installation of items purchased through CIO Systems, Inc. A service call will be credited as a minimum of one hour of labor.
Third party software and hardware support - We will attempt our best to support all third party products (Items not purchased from us), though in some cases we will have to request outside support. We will pass along to you any charges for this outside support.
Since we spend extra time at the start of a contract to fix existing problems, we require a six month commitment, after that time this agreement is month to month. To cancel, after the initial six months, you need to contact us before the end of the month to cancel for the next month.
Though we try to best of our abilities to keep your computer equipment working, we cannot be held liable for any downtime cost, lost profits or another cost or loses. At your discretion we will act as your agent in dealing with the manufacturers of your hardware and software. Any and all warrantees are between you and the manufacturers of your hardware and software. CIO Systems, Inc. offers no warrantee expressed or implied, unless issued in writing and signed by an officer of the corporation
